Located on the 5th floor of the Mercer’s Institute for Successful Ageing (MISA) building.
Transitional Care Ward – 29 beds comprising – 20 single rooms and 3 rooms with 3 bedded capacity. Admissions to transitional care is by direct transfers from hospital wards for patients who have completed their acute phase of care and await long term care placement or discharge home with Home Care supports.
Patients transferring to the transitional ward must have a structured discharge plan in place and a planned discharge date.
